Our eating habits reflect the way we live. Please print this quiz and evaluate the following questions on a scale of one to five. Circle the most applicable answers using the point system:

5=Daily or Always

REMEMBER THAT WE ALL HAVE SITUATIONS WHERE WE EAT OR OTHERWISE PERFORM LESS WELL THAN WE WOULD LIKE AND THAT TOTAL HONESTY WITHOUT JUDGMENT IS THE KEY TO SELF-KNOWLEDGE AND SELF-EMPOWERMENT.
